CareStack provides the flexibility to create and customise forms to ask questions your way. The forms simply need to be added to the patient profile so the patient can complete them either via the Kiosk or the Patient Portal.

To add a form, perform the steps below:
- Select the patient using the Global Search Bar.
- Click on the + Form shortcut on the top panel.
On the Add Form(s) modal,
- Checkmark the intended form(s).
- Select the Clinician for those forms that require a clinician signature.
- The clinician dropdown is filtered based on the form's assigned location, displaying Clinicians associated with that location.
- Click on either Fill Now or Add Forms.
- Fill Now: This option allows you to fill the form out immediately, as it generates a pop-up.
Add Forms: Adding a form to the Forms page will let you fill in the form later at your convenience.
| Note: Users with access to multiple locations will see a similar caution alert while adding forms/letters. |
- Fill out the form with suitable responses.
- If there are any dropdowns, such as Appointment, Clinician and Referral Clinician, select the intended items from the list.
| Note: For forms that require appointment linking, choose the relevant appointment from the dropdown. Future appointments will be displayed at the top of the appointment picker. |
Fill or edit the form contents.

- Select the Relationship to the Patient and Name.
- Click Add Signature to input the required signatures depending on the form template.
- Click Print to take a printout of the filled form.
- You can also print an incomplete form if desired.
- If the signature is yet to be added, the Save as Draft option will appear.
- This allows you to save and complete it later.
- You can print a form without a signature.
- Short answers will be displayed in a single line, whereas long answers will be shown in an expanded format.
- The fields marked as mandatory will be marked with an '*' and (*denotes required) towards the end.
- The signature placeholders, such as Patient Signature, Clinician Signature and Witness Signature, will be visible in the form with a line to denote where to add them.
Click Save once the signature is added to save the changes and add the form to the patient's profile.
| Note: Smaller files added to a form will open instantly in a new tab for quick printing or downloading. If a document takes longer than 3 seconds to process, it will continue in a separate tab, letting you navigate the system and finish other tasks simultaneously. |
- On the Patient Documents page, files will be marked as Processing until they are completed.
- Once a file is uploaded, a toast notification will appear on the top right stating, '<Document_Name> of <Patient_Name> is now available.'
