Managing medical history forms is crucial for patient care in dental practices. It simplifies the process for practices to create and handle these forms. Patients can conveniently update their medical history using the Kiosk and Patient Portal. Additionally, practices can effortlessly access a patient's profile to add a new Medical History Form, which can be completed electronically or on paper.

Add a Medical History Form
Let's explore how to access and manage medical history forms.
Select the patient using the Global Search Bar.
Click Med. Hx. (Medical History) from the left-hand side panel.
- On the top right of the Medical History Forms page:
- Click + Add Form.
- You will have access to a form where you have to fill out the following:
- Conditions
- Allergies
- If the selected patient has a condition or an allergy,
- Click Yes on the corresponding Condition or Allergy from the form.
Note: When you select Yes for a condition or allergy, the line item will be highlighted in red. |
- Select No for the Conditions/Allergies that are not applicable to the selected patient.
- Click I don't know if you are not aware that the patient has a specific Condition or Allergy.
- Click + Note to add any additional information.
- On completion of each section:
- Click Next.
In the Current Medications tab,
- You can add new medications or edit current medications prescribed for the patient.
- To add a new medication, click + Add Medication.
- Enter the Medication Name, Start Date, and End Date.
- Add a Note to highlight any additional information for the medication.
- Click Add once the note has been added.
- You can End Medications or Edit using the pencil icon if needed.
- Once you have added the necessary medications, click Next.
| Note: If you do not have any Conditions, Allergies, or Current Medications, you can select Mark all as No and select Next. |
Once added, click Next.
- Fill in the Dental Questionnaire and Medical Questionnaire.

- Click + Note to add any notes against each question.
- Once you have completed a questionnaire, click Next to proceed to the next section.
Once the form has been filled, it can be completed by either of the two steps given below.
Enter the Signature of Patient and the Signature of Clinician by clicking on Add Signature.
- Add an Electronic signature using your mouse and click Add.
- The form will be completed and finalised. Or,
Click Print and Sign to directly complete the form and take a printout.
Tip: Clicking Print and Sign will allow practices to take a printout of the medical history form and collect the signature physically in a hard copy. |
- On the resulting pop-up, click Proceed to confirm.
- Once you print, you can see the print version of the medical history form with Conditions, Allergies, Current Medications, Questionnaires, and Categories.
Note: You can save the form as a draft by clicking Save as Draft. This will allow you to save the current progress and resume completing the form later. |
- Once the form is completed, you can access it from the left-hand side.
- The line item will display the date when the form was completed.
- You cannot edit a medical history form once it is completed.
- To add a note, click +Note and enter the required information as needed.
- Click Print Blank Form to take a printout of the medical history form without the responses.
| Note: If a form is already added, you cannot add another form, as there can be only one form in pending status. |
- Once a patient completes a medical history form, the practice cannot edit it from the medical history tab, and instead must go to Medical Alerts.
View Medical History
You can view a patient's Medical History on the Patient overview page.
To begin,

Select the patient using the Global Search Bar > click View Medical History Form.
- Clicking on View Medical History Form will take you to the Med. Hx. (Medical History) section.
- The Medical Alerts section includes Conditions, Allergies, Current Medications and Dental and Medical Questionnaires.
- The allergies or conditions are displayed for the selected based on the answers provided on their Medical History Form.
