CareStack supports NHS Complex Care Pathways (CCP), enabling practices to create, manage, and submit CCP treatment plans.

To get started,
- Search for the patient using the Global Search Bar.
- Click the Chart quick link for the intended patient.
On the desired NHS Patient's treatment plan,
- Always ensure to add any exemptions to the plan, if any, before marking them as CCP.
- Select Mark as Complex Care Pathway from the gear (
) icon to the right of the NHS (payor) label.

When a pathway is selected, the Entry Criteria and the following are displayed:
- Number of required submissions
- Total NHS payment for the pathway
- Payment per submission
- Patient payable amount
- Click Confirm to proceed.
- Add and charge out any treatment code to the plan, if required.
- A light purple banner with the computed patient charge will be displayed in the treatment plan.
- Click the Submit Signup button to open the Complex Care Pathway claim pop-up.

This pop-up comprises of all the claims pertaining to that CCP.
- Once the sign up claim is accepted by the NHS, the clinician can submit the succeeding ‘Interim’ claims after reviewing the applicable declarations.

CCP Statuses
CCP plans show one of five statuses: Pending Signup, In-Progress, Completed, Suspended, and Incomplete, visible on both the Claim form and the Chart.
Pending Signup: Default status when a treatment plan is first marked as CCP. No declarations have been submitted yet (Stage 0).
In-Progress: Active pathway, at least one interim declaration has been submitted. Always shown with the current stage, e.g. In-Progress (3/6).
Completed: All required submissions for the pathway are finished (e.g., Stage 6 for Pathway 1 & 3, Stage 12 for Pathway 2).
Suspended: The clinician has submitted a Suspend declaration, temporarily pausing the pathway. It can be re-opened by submitting the next interim. Auto-terminates as Incomplete after 93 days if not re-opened.
Incomplete: The clinician has terminated the pathway early via "Mark as Incomplete." This is irreversible; the patient can only be enrolled in a new pathway after the NHS-mandated minimum interval.
The declaration/claim submission button dynamically adapts based on the CCP status:
- Pending Sign Up → ‘Submit Signup’ button only.
- In-Progress → ‘View Declaration’ with options to Suspend or Mark as Incomplete.
- Completed / Incomplete → ‘View Declaration’ button only.
- Suspended → ‘View Declaration’ with Mark as Incomplete option.
When a CCP status transitions to In-Progress, Completed or Incomplete; fees are recalculated as per NHS rules.
When a CCP plan is Completed, the system automatically recalculates fees for all other open, unsubmitted NHS treatment plans for the patient based on NHS logic regarding Treatment Plans concurrent with CCP.
When the plan status is In-Progress, the system will scan all other open (unsubmitted) NHS plans of the patient and recalculate fees as per the NHS fee logic regarding Treatment Plans concurrent with CCP.
CCP plans in Suspended status will be automatically transitioned to Incomplete after 3 calendar months, with fees recalculated as per NHS rules.
Suspend or Mark a CCP Plan As Incomplete
- Choose Suspend to pause the treatment temporarily and Mark as Incomplete for a permanent closure.
